I got the Olympus PEN leather case which the camera will only fit into when it has a pancake lens on. Are people putting their cameras into these with the strap still on them as well as the strap on the pouch, or something else? It's a bit of a fiddle folding the strap back into it.

Oddly, I can't seem to find the case on the web anywhere to show which one I'm talking about...

Do you have a case like this
    4058129853_02368dfdf1.
    or this?

Then the strap should not go into the case. You can use the strap to carry camera and case around your shoulder, always ready.

If you have a case like this:

Maybe it is most convenient to use with a wrist strap. Then you have your camera on the wrist when you shoot, or in the case around your shoulder for at-hand carrying.
